Schedule:                                      August 2024 – Spring 2025

12-15 hours/week: can be flexibly scheduled between 9:00 am-6:00 pm, M-F

Supervisor:                                   Program Manager/Program Supervisor

FLSA Status:                                 Stipend of $1,500- $2,000 upon successful completion (300 – 400 hrs)

 

Summary: This position supports housing stability, health, and individualized goals with  adults and families living in PPL housing. The position requires the ability to connect with persons from diverse communities who have experienced long-term homelessness, mental and chemical health disabilities, and trauma. Provide case management and direct service focused on helping participants maintain housing stability. Provide community building services. Use a Person-Centered, Harm Reduction, and Housing First approach to build positive relationships residents in their homes or in the community.

Essential Duties and Responsibilities:

  • Meet one-to-one with residents via phone or in-person to identify and connect to community resources and services. 
  • Provide ongoing support and follow-up to a small assigned case load.
  • Assist with resident transportation needs and accompany residents to appointments, food shelf, activities, etc.
  • Organize and facilitate social activities to promote peer support and community involvement.
  • Distribute community resources and information by door knocking.
  • Document and maintain all communication with residents, landlords, and service providers in participant database.
  • Attend weekly staff meetings, team consultation, and race equity trainings.
  • Perform related administrative duties and other duties as assigned.

Qualifications:

  • High level of attention to detail and follow-through
  • Understanding of issues related to mental illness, chemical dependency and homelessness preferred
  • High degree of cultural sensitivity; comfortable working with participants from diverse backgrounds.
  • Valid driver’s license, good driving record and willingness to transport/accompany clients as needed
  • Ability to adhere to HIPPAA and data privacy requirements
  • Must pass criminal background check
  • Excellent communication and interpersonal skills
  • Willingness to work a hybrid schedule with on site, in person hours

Education and/or Experience:

  • Pursuing a degree (or recent graduate) in human services, social work, criminal justice, chemical dependency counseling, psychology or related field.

Physical Demands:

Sitting, bending, and stooping. Occasionally lift and/or move up to 25 pounds. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
