Job Summary

PPL is in search of a Supportive Housing Coordinator II for its Resident Services- Single Adult Supportive Housing Department. This position provides screening and intakes, housing search and placement, rehousing, and landlord recruitment to help single adults find and keep housing in Hennepin and Ramsey Counties.

The ideal candidate will have the skills to build relationships with private landlords and PPL property managers; provide case management (assessment, plan development, and monitoring; referrals; coordination and advocacy with necessary resources and benefits), housing tenancy support, and direct service. The preferred candidate will use a person-centered, harm reduction, and housing-first approach to partner with participants from diverse communities who have experienced homelessness and behavioral health challenges.

This is an excellent opportunity for a BSW or MSW candidate looking to gain practice hours and free supervision toward their license requirements. 

Job duties and responsibilities include:

  • Housing intake, search and placement.
  • Obtain referrals of adults who are long term homeless through Hennepin and Ramsey County’s Coordinated Entry Systems.
  • Keep accurate and timely paperwork and documentation.
  • Develop individualized housing search and placement plans for initial or rehousing placements.
  • Locate new landlords; build positive relationships with private landlords and PPL property managers.
  • Connect households to security deposits, moving costs, furnishings, basic needs and emergency resources.
  • Assist with arranging for and supporting the details of move.  Perform Habitability Inspections and required recertification processes.
  • Transfer tenancy sustaining services and case management to assigned case manager once housing has been secured.
  • Provide tenancy-sustaining services and case management with a small caseload (understanding the lease, communicating with landlord, managing neighbor conflicts, paying rent on time and addressing other behaviors that may jeopardize housing).
  • Provide back-up tenancy-sustaining services and case management to Hennepin County Housing Supports participants in the event of an unexpected staff absence (e.i. unexpected leave, extended PTO, etc.). 
  • Utilize skills and community resources to provide crisis prevention and intervention.
  • Support strategies to promote neighborhood, apartment building, and personal safety.
  • Support participant health and wellness through connections to physical, mental health, and recovery resources.
  • Assist participants to maintain or increase income through benefits assistance or involvement with volunteering, education, and employment activities.
  • Help participants navigate transportation needs, including access to bus tokens, gas cards and driving residents to appointments in personal or PPL vehicle.
  • Work with team and participants to plan and facilitate community building activities, resident meetings, and engagement with the larger community. Help residents build social support with family, peers, neighbors, etc.
  • Provide and/or facilitate translation and interpreting services.
  • Communicate and coordinate services with all team members and other needed internal and external stakeholders.
  • Participate in internal and external professional development, team meetings, case consultation, special initiatives, PPL committees, and organization-wide meetings.

 Education and Experience:

  • Prefer Bachelor’s Degree in Human Services, Social Work, or related field, but Associates Degree and experience with target population may be considered as a substitute for some education. 
  • 3-4 years experience in providing services to individuals or families with behavioral health challenges and/or in a supportive housing environment. Prefer experience in housing search and placement.
  • Self-starter with excellent verbal and written communication skills.
  • Ability to respond to the unique cultural, economic and social needs and resources of residents, using these unique resources to meet their goals.
  • Experience with case management or developing supportive housing service plans.
  • Ability to provide crisis intervention and support.
  • Strong group facilitation, mediation, interpersonal, organizational and outreach skills.
  • Ability to efficiently coordinate, track and complete multiple tasks as well as adjust to changing priorities.
  • Ability to work independently and as a team member.
  • Ability to maintain confidentiality and boundaries in all interactions with residents, staff, volunteers, and the public.
  • Must have a valid driver’s license, a good driving record, proof of insurance and a reliable vehicle.
  • Experience with Motivational Interviewing, Person-Centered, or DBT skills a bonus.
  • Ability to recognize and address mental health challenges, chemical dependency and other problematic behaviors.

Salary: $20.19-$22.12/hr., DOQ
