Job Summary

The Housing Case Manager will provide case management and direct service focused on helping residents maintain housing stability at Adair Campus, Elliot Park Commons and some scattered sites. The position will provide assessment, plan development and monitoring, referrals, coordination, and advocacy with necessary resources and benefits. The Housing Case Manager will use a Person-Centered, Harm Reduction, and Housing First approach to partner with participants from diverse communities who have experienced homelessness, behavioral health challenges, developmental or cognitive disabilities. Build positive relationships by meeting regularly with a caseload of families/individuals in their homes or in the community

PPLs Supportive Housing, using the Housing First philosophy, helps over 200 single adults each year find and keep housing, and reach educational, employment, family and individual goals. Participants have long-term homelessness and live with mental illness and chemical dependency. Housing is located within PPL properties and with private landlords in Hennepin and Ramsey counties.

Additional responsibilities include:

  • Engage with participants to assess their strengths/barriers and create customized participant-driven housing stability plans. Update and modify to reflect current goals, needs and housing retention barriers. Keep accurate, up to date documentation including but not limited to, goal plans, case notes, incident reports, funder data, monthly orders, receipts, etc.
  • Support participants through the housing application process and provide transition services as they prepare to move-in and set up a home. Connect households to security deposits, furnishings, basic needs and emergency resources. Provide tenancy sustaining services to include understanding the lease, communicating with property management, managing neighbor conflict, paying rent on time, and addressing other behaviors that may jeopardize housing. Assist with the housing recertification process.
  • Visit participant apartments regularly to ensure that they are properly maintained. Support strategies to promote neighborhood, apartment building, and personal safety. Work with team and participants to plan and facilitate community building activities, resident meetings, and engagement with the larger community. Help residents build social support with family, peers, neighbors, etc. Provide and/or facilitate translation and interpreting services.
  • Utilize skills and community resources to provide crisis prevention and intervention. Support participant health and wellness through connections to physical, mental health, and recovery resources. Assist participants to maintain or increase income through benefits assistance or involvement with volunteering, education, and employment activities.
  • Help participants navigate transportation needs, including access to bus tokens, gas cards and driving residents to appointments in personal or PPL vehicle.
  • Communicate and coordinate services with all team members and other needed internal and external stakeholders.

Minimum Requirements

  • Ability to work late afternoon hours several days each week and one early evening monthly.
  • Self-starter with excellent verbal and written communication skills.
  • Ability to respond to the unique cultural, economic and social needs and resources of residents, using these unique resources to meet their goals. Strong group facilitation, mediation, interpersonal, organizational and outreach skills.
  • Experience with case management or developing supportive housing service plans.
  • Ability to efficiently coordinate, track and complete multiple tasks as well as adjust to changing priorities. Ability to work independently and as a team member.
  • Ability to maintain confidentiality and boundaries in all interactions with residents, staff, volunteers, and the public.
  • Experience with Motivational Interviewing, Person-Centered, or DBT skills preferred.
  • Ability to recognize and address mental health challenges, chemical dependency and other problematic behaviors. Ability to provide crisis intervention and support.
  • Must have a valid driver’s license, proof of current insurance, a good driving record as determined by our insurance carrier, and a reliable vehicle

Education & Experience

  • Bachelor’s Degree in Human Services, Social Work or a related field preferred; an Associate’s Degree and experience with target population may substitute for some education.
  • Three to four years of experience providing services and case management to individuals or families with behavioral health challenges and/or in a supportive housing environment. Experience in housing search and placement preferred.

Salary: $19.23-$20.19/hr., DOQ
